Diabetes Free SC (DFSC) has formed working partnerships with groups and programs across the state that align with and buttress our strategic directions. Explore the roster of partners on this page.

Diabetes Free SC seeks to align our activities with the following organizations and initiatives:

BlueCross BlueShield of South Carolina Foundation (the Foundation)

The BlueCross® BlueShield® of South Carolina Foundation issues grants to support projects that aim to promote the health of South Carolinians and to increase access to health care for the economically disadvantaged. The Foundation was part of the original Diabetes Free SC planning group and provides funding for Diabetes Free SC programs and initiatives. The BlueCross BlueShield of South Carolina Foundation is an independent licensee of the Blue Cross Blue Shield Association.

Alliance for a Healthier South Carolina (Alliance)

The Alliance for a Healthier South Carolina is a coalition of more than 50 executive leaders from diverse organizations across the state working together to ensure that all people in South Carolina have the opportunity to have healthier bodies, minds and communities while reducing the future cost of care.

South Carolina Department of Health & Environmental Control (DHEC)

The Department of Health and Environmental Control’s Diabetes and Heart Disease Management and Prevention Division supports state investments in implementing and evaluating evidence-based strategies to prevent and manage diabetes in high-burden populations. DHEC is a key partner of Diabetes Free SC in the development of diabetes prevention strategies and initiatives. It also assists with the alignment of community partner efforts across the state. DHEC was part of the original Diabetes Free SC planning group.

Diabetes Initiative of South Carolina (DSC)

The Diabetes Initiative of South Carolina coordinates efforts to identify and manage diabetes. The DSC provides diabetes management tools and educational programs with the goal of reducing complications and cost burdens for South Carolinians who suffer from this chronic disease.

South Carolina Hospital Association (SCHA)

The South Carolina Hospital Association is committed to making South Carolina one of the nation's healthiest states by helping our hospitals and health systems provide the best care possible. SCHA serves as a collective voice for the state's hospital community and connects their members to local and statewide health improvement initiatives. We work together to ensure that Diabetes Free SC initiatives are informed by SCHA goals and aligned with their efforts to improve health. 

South Carolina Birth Outcomes Initiative — Access & Care Coordination Workgroup

The South Carolina Birth Outcomes Initiative is an effort by the South Carolina Department of Health and Human Services, South Carolina Hospital Association, BlueCross BlueShield of South Carolina, South Carolina Department of Health and Environmental Control, March of Dimes, and over 100 stakeholders to improve the health outcomes for all moms and babies.

 

BlueCross BlueShield of South Carolina is an independent licensee of the Blue Cross Blue Shield Association.
