Adult Clinical Care Initiative

The Adult Clinical Care Initiative supports Diabetes Free SC’s goal of preventing diabetes and its complications by strengthening care at the clinical level. It brings together care teams made up of providers, diabetes educators, dietitians, case managers and community health workers (CHWs). These teams coordinate to support patients and improve key health measures such as blood sugar, blood pressure, cholesterol, and weight. The initiative also encourages patients to stay engaged in regular, preventive care.

This initiative builds on the success of prior DFSC programs including the Women’s Health Initiative (2021 – 2026) and the Community Health Worker Initiative (2022 – present). These programs have shown that when clinical care is paired with community-based support, patients experience better outcomes and report a better care experience. CHWs help patients navigate barriers, connect to resources and stay on track with their care plans, while also giving providers insight into the challenges patients face day to day.

The initiative aims to raise the standard of diabetes care across South Carolina. It sets clear expectations for care and tracks results at the patient level to measure progress. By combining strong clinical care with the proven impact of CHWs and multidisciplinary teams, it is designed to help more adults better manage their diabetes and reduce complications over time.
